Aggregate Accommodation
The purpose of this provision is to help the contract holder with funding assistance.  The following terms and conditions apply to this provision.

• USB allows one advance per month subject to a minimum of $1,000.
• Aggregate premium must be paid current.
• Checks issued by the Plan must be released to the payees prior to the request.
• All claims eligible for aggregate accommodation must be identified.
• The request must include the applicable list of payments.
• Advanced funds must be paid directly to the payee.
• All checks issued for claim payment must be verifiable.
