Large Claim Notification
The TPA must inform USB of any and all large claims. Large claims include prior shock claims and ongoing claims. Please include the following for prior shock claim and ongoing large claims where appropriate.
- Historical, pending and potential claim paid amounts and dates paid by claimant
- Current status of claimant; active, disabled, terminated, retired, or COBRA
- Provide prognosis, diagnosis and large case management notes
- Pended and denied claim reports by claimant
