Premium Payment
Premium payments are due on the first of the month.  The contracts are on a self bill basis, no billing statements are sent.  USB will notify the TPA when we have not received a contractholder’s premium by the 15th of the month.  The premium is due by the end of the grace period.  If premium has not received by the end of the grace period, the Contract will be terminated or rescinded without further notice.

Premium Remittance Summary.xls contains the information USB will need to process the premium payment.
