USB respects the confidentiality of medical information and will protect that information in a responsible manner. USB has a comprehensive privacy program in place that limits access to personal medical information to only those persons who need to know said information to provide the products and services which USB manages. These products and services are inclusive of, but not limited to self funded and fully insured employee benefit insurance products. These products require certain persons to have access to medical information who are trained and monitored on the appropriate safeguarding of this sensitive material.
Confidential medical information may mean information that is individually identifiable health information. Usually this information is assigned a unique identifier as part of a safeguarding process. This may include information that is submitted to USB from a third party administrator, health care provider, a health plan, utilization review vendor or consultant appointed by an employer group. USB limits the collection of confidential medical information to that which is necessary to manage, service and administer in-force and prospective business opportunities. The collection of confidential medical information is for the use of and under the auspice of Health Care Operations.
Before making a significant change to the company’s privacy practices, this notice may be changed. This new notice will be effective for all medical information that is maintained, including medical information created or received before the changes were made.
